Position Summary
Lincoln County Public Health is looking for a highly motivated individual to be a part of a diverse team to provide healthcare services to pregnant and postpartum clients and their babies in our community. This person will work in a community setting, providing quality nursing care in Public Health's General Field program, this includes Babies First! and CaCoon. This role will work for the health of pregnant and postpartum clients and their babies in Lincoln County through home visiting services. This nurse will use the standards of practice for community health nurses and assigned program models as guidelines to practice.
$40.19 - $54.02 per hour* *Lincoln County follows Oregon Pay Equity laws in reviewing education and experience for wage offer.

EDUCATION An Associate's Degree in Nursing with at least 2 years of experience in a Maternal Child Health home visiting program.
EXPERIENCE Two- Three years experience in nursing. Substitution Note: Practicums in nursing programs and lived experience may substitute for years of experience. LICENSES Oregon Driver's License required within 30 days of hire. Must possess a current unencumbered license to practice in the State of Oregon as a Registered Nurse at time of hire. Desirable Qualifications: Bachelor of Science degree in Nursing (BSN) from an accredited school of nursing with one year of nursing experience, preferably in a maternal child health position. Bilingual and/or bicultural competencies for the Latino population are preferred.

The County's current contribution rate is 11%.
Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.

- Assessment - Collects comprehensive data related to the health of the population served. Document assessment data in terms that are understandable to all involved in the care process.
- Diagnosis and Priorities - Analysis of the assessment data to derive a nursing diagnosis and priorities. Document diagnoses or concerns in a manner that facilitates the client's participation in developing a plan of care.
- Outcome identification - Based on the diagnosis the CHN2 will identify outcomes that are culturally relevant, health literate, based in evidence and attainable through local resources and policies. Document expected outcomes and measurable objectives. Participate in and leads quality improvement measures.
- Planning - Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes. Document the plan using health literate and culturally competent language that can be understood by care team and the population being served.
- Implementation - The CHN oversees the implementation of the plan through treatment, case management, referrals to community partners, collaboration with community resources, health promotion and education.
- Document activities and status of care coordination.
- Evaluation - Apply on-going assessment data to revise plans, interventions, and activities as appropriate for ultimate health outcomes for the population. Document changes to enhance effectiveness of interventions.
- Apply theories and principles integral to the Babies First and CaCoon programs.
- Use the nursing process to deliver individualized client care by means of home, telehealth, office, or hospital visits, provide professional Community Health nursing services in areas such as Maternal, Child & Family Health family planning, and communicable disease control. Assist in planning and operating clinics as needed.
- Provide ongoing case management to at-risk clients such as pregnant clients, clients using substances and high-risk infants. As part of case management functions, observes family dynamics as well as assesses food, shelter and transportation needs.
- Set personal schedule, track clients who are often very difficult to find, and perform visits on a drop-in basis if needed.
- Meet training requirements and practices with fidelity to the assigned programs.
- Establish therapeutic relationships with clients.
- Participate in on-going quality improvement and reports from data systems to guide and improve practice.
- Use reflective supervision to evaluate and improve practice and takes part in self-care activities.
- Participate in epidemiological investigations in homes, schools, or other locations in the community; order immunizations and tests as needed and make recommendations to prevent and control the spread of disease.
- Interact and convene with community partners and resources as necessary to assure population needs are being met.
- Attend and participate in team meetings, trainings, and activities to further the health of the community.
