Reports to Director of Programs

MINIMUM REQUIRMENTS:

• Minimum of AA degree.
• Two (2) years' experience in a related field
• Two (2) years or more as resident in Lafayette and surrounding Counties and working knowledge of community resources and events.
• Proficient computers skills, including Microsoft, specifically Word and Excel, in addition to excellent oral and written communication skills.
• Must have a valid Mississippi Driver's License, proof of insurance and reliable transportation

RESPONSIBILITIES:

• Conduct individual home visits with the participants working on losing weight and changing mindset.
• Manage caseload (up to 75), keeping accurate records and ensuring that all related administrative tasks are completed and up to date.
• Provide the evidence-based curriculum and emotional support.
• Maintain an effective support service for participants with the aim of enabling participants to lose weight and helping with any resources necessary to help their develop of a healthier lifestyle.
• Design and participate in recruitment activities especially promoting increased awareness of services by educating the public and linking with other programs.
• Maintain confidentiality of all participants; communicate orally and in writing to parents, professionals,
• and other agencies.
• Plan and implement activities and educational classes for participant that are culturally sensitive and developmentally appropriate both in individual and group settings.
• Other duties as assigned.
