Palliative Care Nurse Practitioner

Overview
We are seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our healthcare team in Lafayette. LA. This role is essential in providing comprehensive care to patients with serious illnesses, focusing on improving quality of life through symptom management and support. The ideal candidate will possess strong clinical skills and a deep understanding of palliative care principles, ensuring that patients receive holistic and patient-centered care.

Responsibilities

* Conduct thorough diagnostic evaluations and assessments of patients to develop individualized care plans.
* Collaborate with interdisciplinary teams to coordinate patient care, including discharge planning and follow-up services.
* Provide education and support to patients and their families regarding treatment options, symptom management, and end-of-life decisions.
* Manage complex cases involving critical care
* Utilize electronic health record systems for documentation and patient management.
* Participate in ongoing professional development to enhance skills in palliative care practices.

Qualifications

* Masters degree in Nursing with a focus on Palliative Care or related field.
* Current Nurse Practitioner licensure with certification in Palliative Care preferred.
* Strong clinical experience in critical care or related specialties.
* Proficiency in diagnostic evaluation techniques and discharge planning processes.
* Familiarity with electronic health record systems.
* Excellent communication skills, with the ability to empathize with patients and families during challenging times.
* CDS and DEA License required.
* Palliative or Hospice Care experience preferred.
