Nurse Care Manager & Health Education Lead

Samuel U Rodgers Health Center is seeking a Nurse Educator to plan, organize, and direct health education programs for Health Home patients. You will educate patients and families about chronic disease, treatment options, and lifestyle changes, and collaborate with the health home team to set measurable health goals. You will also monitor patient progress, integrate behavioral health into primary care, and employ motivational interviewing to support self-management strategies aligned with #J-18808-Ljbffr Samuel U Rodgers Health Center
