Responsibilities include : Complete skilled nursing visits during evenings and weekends Perform: Start of Care (SOC) assessments Resumption of Care visits PRN visits Coordinate care with: Case managers Interdisciplinary care teams Provide skilled clinical care including: Wound care Disease management education IV starts Support urology-related patient needs including : Urinary retention monitoring Post-surgical urology follow-up Complete documentation within required EMR timeframes Schedule Structure Monday – Friday 4:00 PM – 10:00 PM Coverage may include: Start of Care admissions Late-add visits Productivity Expectations Minimum weekly productivity: 24 points Point opportunities include: Start of Care admissions Skilled nursing revisits PRN urgent visits Resumption of Care visits Qualifications Active Florida RN license or Compact RN license Preferred experience: Home Health Urology Med-Surg Hospice Acute Care Strong assessment and triage skills OASIS documentation experience required Ability to work independently Reliable transportation, valid driver's license, and auto insurance Ideal Candidate You may be a great fit if you: Prefer flexible evening work(4:00 PM - 10:00 PM) Enjoy independent clinical decision-making Feel comfortable managing urgent patient needs Have strong Foley/urology management experience Join a team that values your time, supports your growth, and empowers you to provide exceptional patient care.
