Connected Health Family Care is seeking a Physician Assistant and/or Nurse Practitioner to become a part of our team! You will focus on providing high quality patient care as part of a team based approach to healthcare. We are a growing company looking for qualified individuals to succeed with us!

 

Responsibilities:

• Diagnose and treat acute, episodic or chronic illnesses
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with Supervising Physicians on Staff
• Document, Code and Refer patients as needed.

Qualifications:

• Previous experience in nursing or other medical fields
• 2 years experience in Primary Care Setting
• Ability to build rapport with patients
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership qualities
• Spanish Speaking ( Required )
• EMR Experience, eCinicalWorks is a plus!
• Experience working in Managed Care environments, serving Med-Cal and Medicare Populations.
