Core Values Passion to Serve Be Your Best Bring Your Best Do the Right Thing Do What it Takes Role Overview: The Clinical Nurse Manager is a field-based nursing leader responsible for the clinical performance, coordination, quality, and continuity of care across an assigned Colorado territory. This role functions as an extension of the Director of Nursing, combining people leadership, clinical case management, field-based nursing expertise, quality and compliance oversight, patient advocacy, and new patient admission support. The Clinical Nurse Manager maintains visibility in the field while leading through data, clinical judgment, and accountability to ensure patients receive the right level of care at the right time. This leader is expected to anticipate clinical and operational barriers, deploy resources effectively, coach field clinicians, drive timely resolution of patient‑care concerns, and maintain strong ownership of patient outcomes, staff performance, productivity, and regulatory readiness. 5 Major Job Functions Lead, Manage, and Drive Accountability for Clinical Field Staff Clinical Case Management and Coordination of Care Clinical Quality, Compliance, and Patient Outcomes Clinical Productivity, Direct Patient Care, and Field Operations New Patient Admissions and Interdisciplinary Collaboration Key Responsibilities and Duties: Lead, Manage, and Drive Accountability for Clinical Field Staff Provide day‑to‑day clinical leadership, coaching, mentorship, and accountability for nurses and home health aides within the assigned territory. Serve as an accessible clinical resource for field staff, providing guidance on patient care, clinical decision‑making, documentation, and escalation needs. Monitor staff performance and adherence to Plans of Care, clinical standards, agency policies, and evidence‑based practices. Conduct required supervisory visits and field observations, identify competency or education gaps, and coordinate appropriate training. Complete performance reviews for assigned direct reports and participate in performance management and corrective action processes as appropriate. Review and manage PTO requests for direct reports in accordance with operational and patient‑care needs. Foster a culture of accountability, responsiveness, clinical excellence, and patient‑centered care. Clinical Case Management and Coordination of Care Serve as the primary clinical case manager for assigned patients and maintain ownership of clinical coordination throughout the patient lifecycle. Coordinate nursing, home health aide, physician, community, advocacy, and interdisciplinary services to ensure continuity and quality of care. Develop, implement, evaluate, and update individualized Plans of Care based on patient needs, preferences, goals, and clinical status. Monitor changes in condition, clinical risks, service delivery, utilization, and gaps in care; promptly escalation concerns and drive resolution. Perform required 60‑day supervisory visits for LPNs and 14‑day supervisory visits for HHAs. Perform case management services and Plan of Care development and coordination for new patients during admission to agency services. Clinical Quality, Compliance, and Patient Outcomes Ensure compliance with applicable federal and Colorado requirements, Department of Labor requirements, professional nursing standards, and agency policies and procedures. Maintain continuous survey readiness and ensure clinical documentation is accurate, complete, timely, and compliant. Ensure ordered services and frequencies are delivered according to patient needs and the Plan of Care. Participate in quality assurance and performance improvement initiatives and analyze clinical data to identify trends, risks, and opportunities for improvement. Review and support resolution of adverse events, hospitalizations, falls, complaints, medication concerns, and other significant clinical events as assigned. Partner with clinical leadership to implement corrective actions and prevent recurrence of identified clinical deficiencies. Direct Skilled Nursing Care and Timely Documentation Manage clinical productivity through an intentional balance of direct care, patient engagement, care coordination, and administrative leadership responsibilities. The Clinical Nurse Manager is expected to maintain a minimum of 15 billable patient hours per week, unless otherwise directed based on census, geography, market, or operational needs. Billable patient hours include skilled nursing services, assessments, and other reimbursable clinical care delivered in accordance with the Plan of Care. Patient‑facing time includes required supervisory visits, in‑home clinical support, patient and family education, care conferences, and other direct interactions necessary to support safety, stability, continuity, and quality of care. Patient appointment time includes attending or coordinating medically necessary physician, specialist, or other healthcare appointments when Clinical Nurse Manager participation is appropriate to advocate for the patient, facilitate communication, and ensure continuity of the Plan of Care. Administrative and clinical management time is reserved for documentation, chart review, care‑plan development and updates, physician communication, staff coaching and supervision, clinical escalations, quality and compliance activities, interdisciplinary coordination, scheduling or coverage problem‑solving, and other leadership responsibilities necessary to effectively manage the assigned patient population and clinical team. Productivity is evaluated across the totality of these responsibilities, with an expectation that time is purposefully allocated, accurately documented, and aligned to patient needs, clinical priorities, and organizational outcomes. The Clinical Nurse Manager remains available to provide urgent or high‑priority field coverage, participates in the clinical on‑call schedule as assigned, and proactively intervenes when patient deterioration, safety risk, or increased hospitalization risk is identified. New Patient Admissions and Interdisciplinary Collaboration Complete and/or coordinate initial nursing assessments and Starts of Care for new patients and ensure clinical services are established timely. Develop initial Plans of Care and ensure a smooth transition from admission into ongoing case management. Coordinate with Authorized Representatives and advocacy groups involved in Department of Labor cases. Partner with Intake and Authorization, Scheduling, Operations, Advocacy, and Clinical Leadership to remove barriers to admission and continuity of care. Identify staffing or clinical‑capacity barriers that may delay admission or service delivery and elevate concerns with recommended solutions. Attend clinical leadership and interdisciplinary meetings and communicate meaningful patient, staffing, quality, and operational updates. Assess and manage crisis situations proactively and coordinate intensive support across departments when necessary to protect patient safety and well‑being. Education and Experience Qualifications: The Clinical Nurse Manager must demonstrate the clinical expertise, leadership capability, judgment, and field readiness necessary to independently manage an assigned patient population and support clinical staff throughout Colorado. Required Qualifications: Active, unrestricted Registered Nurse (RN) license in the State of Colorado. Associate Degree in Nursing (ADN) or higher from an accredited nursing program. Minimum of 2-3 years of professional nursing experience. Current Basic Life Support (BLS) certification. Strong clinical assessment, critical‑thinking, prioritization, and problem‑solving skills. Ability to manage competing clinical priorities independently in a field‑based environment. Proficiency with electronic health records (EHRs) and clinical documentation systems. Valid driver's license and reliable transportation for travel throughout the assigned Colorado territory. Ability to participate in on‑call responsibilities and accommodate occasional evening, weekend, or holiday needs based on patient care requirements. Ability to successfully complete required background check and drug screening. Preferred Qualifications: Bachelor of Science in Nursing (BSN). Three or more years of experience in home health, community health, case management, or a related field. Previous nursing leadership, supervisory, charge nurse, clinical coordination, or team‑lead experience. Experience managing complex or chronically ill patient populations in home or community‑based settings.
