LPN/LVN - LTC
We are seeking a dedicated LPN/LVN to join a reputable Long-Term Care (LTC) facility. The ideal candidate should have strong clinical skills, excellent bedside manner, and the ability to work as part of a multidisciplinary care team to provide safe, compassionate, and high-quality care to residents.
Key Responsibilities
- Provide direct nursing care to residents in accordance with facility policies and state regulations.
- Administer medications, treatments, and injections as prescribed.
- Monitor patient conditions, record vital signs, and report any changes to the RN or physician.
- Assist with activities of daily living (ADLs) and support resident comfort and mobility.
- Maintain accurate and timely documentation in medical records.
- Collaborate with RNs, CNAs, and other healthcare staff to ensure coordinated care.
- Provide emotional support and build trust with residents and their families.
- Adhere to infection control, safety protocols, and facility guidelines.
Requirements
- Active LPN/LVN license (State of Massachusetts).
- Minimum 1 year of LPN/LVN experience, preferably in Long-Term Care.
- Current BLS certification (AHA preferred).
