Location: Avera Behavioral Health Center - Sioux Falls Worker Type: Regular Work Shift: Rotating days/evenings/nights/weekends/holidays Pay Range: $24.50 - $33.75 per hour (actual rate depends on experience) Position Highlights All 2026 new grads welcomed to apply Adolescent program expanding to 40 units Child unit: 14 bed unit Adolescent Residential: 8 bed unit Opportunity to meet 70+ coworkers Opportunity to change lives of adolescents and children 1 LPN for 7-8 patient ratio Self‑scheduling program A Brief Overview The LPN practices under the supervision of an RN, APRN, licensed physician, or other healthcare provider. The LPN organizes and directs aspects of care for which they are accountable; communicates and coordinates care with a well‑defined nursing diagnosis and all health team members. What you will do Collect data and conduct focused nursing assessments of patients’ health status. Participate with other healthcare providers in development and modification of patient‑centered care plans. Implement nursing interventions within the patient‑centered care plan. Assist in evaluating responses to interventions. Provide maintenance of safe and effective nursing care rendered directly or indirectly. Advocate for the best interest of patients. Communicate and collaborate with patients and healthcare team members. Assist with health counseling and teaching. Delegate and assign nursing interventions to implement the plan of care. Essential Qualifications Must be able to work the specified hours. Must be able to perform each essential job function satisfactorily. Have visual acuity adequate to perform position duties. Required Education, License/Certification, or Work Experience Licensed Practical Nurse (LPN) – Board of Nursing. Active license in the state of practice upon hire.
