**Required Experience:**- 1+ years of PT (Physical Therapy) Experience- Outpatient Experience**Required Licensure and Certifications:**- Active Illinois/IL Physical Therapist License- Active American Heart Association BLS (Basic Life Support)**Required Skills and Qualifications:**- Strong experience administering a wide range of physical therapy techniques- Effective communication and strong interpersonal skills**Submission Requirements:**- Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)- Work history- Skills Checklist, current to 1 year, pertinent to the specialty (Physical Therapy)- 2 Supervisory Reference Forms Completed with dates employed listed**Certifications:**- BLS (American Heart Association)**State Licenses:**- Illinois
