Position Overview SCOPE OF ROLE : The Substance Use Counselor provides person-centered, recovery‑oriented, trauma‑informed substance use counseling services, including integrated care for individuals with co‑occurring conditions, within a Certified Community Behavioral Health Clinic (CCBHC). Services are delivered through multidisciplinary team collaboration and care coordination in compliance with SAMHSA CCBHC criteria and NYS OMH and OASAS regulations. ESSENTIAL DUTIES & RESPONSIBILITIES Provide outpatient substance use counseling using evidence‑based practices. Deliver integrated treatment for individuals with co‑occurring substance use and mental health conditions. Conduct substance use screenings, assessments, and reassessments within scope of practice. Contribute to and implement person‑centered treatment plans, updating as participant needs change. Participate in multidisciplinary treatment team meetings and care coordination activities. Support crisis response, safety planning, and continuity of care, including participation in on‑call rotation as assigned. Complete timely, accurate documentation in the EHR in accordance with OMH, OASAS, Medicaid, and CCBHC standards. Advocate for participant rights, cultural responsiveness, and recovery‑oriented care. Participate in required trainings, supervision, and quality improvement activities.
