LPN - Patchogue, New York

Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Swan Lake Nursing & Rehabilitation in Patchogue, New York. As an LPN, you will play a vital role in delivering high-quality patient care, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

• Provide a full range of healthcare services, including assessment and treatment of common acute and chronic health problems, in accordance with current standards of nursing practice

• Supervise, coordinate, and evaluate the health care team and services on the unit

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Provide guidance and support to subordinate personnel

• Maintain accurate and detailed records of patient care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current licensure as a Licensed Practical Nurse in the State of New York

• Graduate of an approved NLN accredited nursing program or State Approved Vocational Program

• Ability to provide leadership and supervision to subordinate personnel

• Genuine interest in gerontology and an understanding of the special needs of the geriatric elder

• Successful completion of pre-employment health screening and background check

• CPR certification

Additional Benefits:

• $38 - $40 HOURLY salary range
