Resident Medical Coordinator for Disability Services

ACLD in Nesconset, NY is seeking a Medical Coordination Specialist to provide medical coordination and support for people with intellectual and developmental disabilities at a residential site. You will advocate for residents, promote health and safety, and mentor in independent living skills as outlined in each Life Plan. Responsibilities include overseeing medication administration, coordinating weekly nursing reviews, and arranging medical appointments with families and nurses. #J-18808-Ljbffr Acld
