MDS - LPN/RN

General Purpose The primary purpose of this position is to oversee the facility to ensure assessments are completed according to guidelines. The role focuses on the resident assessment process, including individual assessment, MDS assessment, and resident plan of care. Essential Duties Provides oversight on facility’s weekly PDPM meetings for skilled patients. Tracks and monitors facility effective utilization of PDPM tools (including Physician Diagnosis Verification, Admission/Discharge CSR, Weekly CSR, PDPM Weekly Tracking Tool, Point Right). Tracks and monitors QM and QASP performance monthly and quarterly, and provides data analysis and clinical recommendations for optimal outcomes. Provides PDPM and MDS training to new MDS Coordinators. Assesses and determines the health status and level of care of all new admissions. Ensures accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change. Communicates level of care for new residents to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to the facility according to regulatory time frames. Maintains an accurate schedule of all MDS assessments with proper reference dates throughout the resident’s stay. Responsible for data entry to ensure accurate electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary, and maintains appropriate records. Schedules and conducts resident care conferences in compliance with state and federal regulations, ensuring completion of all MDS reviews prior to the conference. Assists disciplines in formulating and revising care plans, ensuring problems are identified, goals established, and nursing interventions appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes, ensuring the care plan is reassessed and revised appropriately. Manages all level of care changes within the facility and notifies all departments when a change has been made. Generates appropriate forms to complete level of acuity changes and transmits them to the appropriate agency as required by state law. Maintains confidentiality of all resident care information, including protected health information. Supervisory Requirements This position includes supervisory duties. Qualifications Graduation from an accredited nursing school with a Bachelor of Science Degree in Nursing preferred. Current good standing with the State Board of Nursing and completion of all required continuing education/licensing requirements. Minimum of 3 years of supervisory experience preferred. Long‐term care experience preferred. Proficiency in Microsoft Suite products; experience with PCC preferred. Must remain in good standing with the State Board of Nursing at all times. Physical Demands The employee must occasionally lift or move up to 25 pounds, use a desktop or laptop computer for prolonged periods, sit, stand, walk, talk, read, or hear. Frequent use of office equipment (copier, scanner, fax, telephone, calculator) and travel by auto or airline may be required.
