Physical Therapist, General — Clearlake, CA. Travel - Temporary Assignment starting 09/28/2026 for 13 weeks. Weekly pay range: $2,615–$2,693 per week with 40 guaranteed hours. Shift: Day shift, 5x8 (Mon–Fri). Setting: a 25-bed critical access hospital. Key Highlights Weekly pay: $2,615–$2,693 per week Guaranteed hours: 40 per week Contract duration: 13 weeks Shift: Day shift, 5x8 (Mon–Fri) Start date: 09/28/2026 Setting: 25-bed critical access hospital in a hospital-based PT assignment Responsibilities Conduct patient evaluations and develop individualized treatment plans to restore and improve physical function Implement evidence-based PT interventions and adjust plans based on patient progress Complete comprehensive documentation, including assessment findings, treatment plans, progress notes, and discharge summaries, within established deadlines Collaborate with an interdisciplinary care team and co-lead patient groups as appropriate Supervise assigned therapy assistants and communicate discharge or follow-up needs to patients, families, and care team Requirements Doctor of Physical Therapy (DPT) degree (or foreign equivalent) when applicable; license in the state(s) of practice Current physical therapy licensure in the state(s) of practice and eligibility to practice in California for this assignment CPR certification or BLS (American Heart Association) required; additional BLS/CPR variants accepted per policy Valid Driver’s License and must be at least 21 years of age Must know HCHB and OASIS; EPIC experience; at least two years of recent PT experience; 1 year of home health experience preferred Ability to work in a hospital-based setting; no weekends; day shift; 40 hours per week Benefits and Support 24/7 recruiter and travel support Dedicated assignment support throughout the contract Mileage reimbursement paid at the current government rate Consistent weekly pay and guaranteed hours Please note: Pay rates and guaranteed hours are estimates and may vary.
