Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness of care.
- Document, prepare, and review patient care plans, including progress notes and clinical record documentation.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals.
- Instruct patients and families on transfer techniques and the use of equipment such as walkers and wheelchairs.
- Educate patients and families regarding therapeutic procedures based on individual factors including age and condition.
- Collaborate with an interdisciplinary team to provide comprehensive patient care.
