• Requirements

• Previous Home Health PT experience required
• OASIS experience required
• HomeCare HomeBase (HCHB) experience required
• Current West Virginia PT license required
• BLS certification required
• Strong time management and independent clinical skills

• Responsibilities

• Evaluate and treat patients in the home setting
• Complete OASIS assessments and point-of-care documentation
• Document daily visits using HomeCare HomeBase
• Develop individualized treatment plans focused on safety, mobility, and independence
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary teams
• Educate patients and families on mobility, fall prevention, and home safety
