University Home Care Inc is seeking an Experienced Physical Therapist Homecare to join our dedicated team in Livonia, MI. In this role, you will provide high-quality, personalized physical therapy services to patients in their homes, helping them recover, improve mobility, and maintain independence. You will use your expertise to develop and implement treatment plans tailored to each patient's specific needs. Responsibilities Conduct thorough patient evaluations and develop individualized treatment plans Provide hands‑on physical therapy treatments in the patient's home setting Monitor patient progress and adjust treatment plans as necessary Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care Document patient care and maintain accurate records in compliance with company policies and regulatory standards Educate patients and their families about exercises, recovery processes, and injury prevention Requirements Valid Physical Therapist license in the state of Michigan Proven experience in homecare or outpatient physical therapy settings Excellent interpersonal and communication skills Ability to work independently and manage time effectively Reliable transportation to visit patients’ homes Current CPR certification Benefits Competitive pay rate of $70.00 - $80.00 per visit, paid bi‑weekly Flexible scheduling options Supportive team environment Opportunities for professional development and continuing education About the Company University Home Care Inc is a leading provider of home healthcare services dedicated to improving patient outcomes and enhancing quality of life.
