Now Hiring Experienced Home Care Visit Nurse, Per Diem in St. Cloud Visit Pay between $60-70/Visit BrightStar Care, a leader in home care, is currently seeking compassionate and dedicated Licensed Practical Nurses (LPNs) or Registered Nurses (RNs) to join our team. Traveling to a client's home within our territory is a requirement (St. Cloud) We are just expanding to the St. Cloud Community.

Key Responsibilities:

• Provide nursing services to patients in a home setting.
• Services may include, but not limited to; wound care, INR, Medication Management, Catheter Cares, etc.
• Perform routine nursing care to patients as prescribed by the healthcare provider.
• Monitor patient health and maintain accurate records of vital signs and health progress.
• Educate patients and their families on health maintenance and disease prevention
• Document Nurse Visits in accordance with State and Joint Commission Guidelines.

Requirements:

• Valid LPN or RN license in the state of practice. (Minnesota)
• Experience in home health care or similar settings is preferred.
• Strong interpersonal skills and a passion for providing empathetic, respectful care.
• Ability to work independently and efficiently with keen attention to detail.
• Ability to Travel and Commute throughout Territory (Maple Grove, Elk River, Anoka, Circle Pines and Surrounding Areas)
• Reliable Vehicle, Current Auto Insurance, and Valid Driver's License
