Roles & Responsibilities

• Provide top-notch physical therapy services to residents in a skilled nursing facility
• Develop and implement individualized treatment plans to improve mobility and function
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Document patient progress and adjust treatments for optimal outcomes
• Educate patients and families on therapeutic exercises and home programs

Qualifications

• Active Physical Therapist (PT) license in Minnesota or compact eligibility
• Doctor of Physical Therapy (DPT) degree preferred
• New graduates welcome to apply - we support your growth!
• Previous SNF experience is a plus but not required

Knowledge & Skills

• Expertise in patient assessment and treatment planning
• Strong communication and teamwork abilities
• Knowledge of rehabilitation techniques for geriatric populations
• Proficiency in electronic medical records (EMR) systems
