Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title Physical Therapist Location Florida Schedule / Job Type Contract | Full-Time | Days | Monday–Friday, 40 hours per week | Onsite Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Delegate and direct all discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements Required Qualifications Active Physical Therapist licensure in good standing in the state of Florida Minimum of 1 year of clinical experience as a Physical Therapist Experience conducting 60-minute evaluations and treatments in an outpatient setting Proficiency in medical record documentation, goal writing, and compliance with state and federal guidelines Demonstrated commitment to patient safety and adherence to professional codes of conduct Preferred Experience / Skills Experience in an outpatient rehabilitation environment Familiarity with performance improvement methodologies and quality of care initiatives Active involvement in professional development activities, continuing education, or professional organizations Why Join Us Work in a collaborative, patient-centered environment that prioritizes both patient and employee safety Opportunity to contribute to a structured professional development program with ongoing clinical growth support Concentric Healthcare Staffing Job ID 765262.
