More About Our Team This 60‑Bed Hospital serves patients recovering from strokes, brain and spinal‑cord injuries, amputations, complex orthopedic injuries, and other conditions. Flexible Hours Per Diem, 8‑Hour Day Shifts Opportunity for Growth How You'll Contribute The Per Diem Outcomes Manager coordinates timely and accurate data completion of the Inpatient Rehabilitation Facility‑Patient Assessment Instrument (IRF‑PAI) for each rehabilitation inpatient. Collects, reviews, and monitors the information in the IRF‑PAI for each patient, assuring accuracy in scoring functional status and reflecting the patient's care needs as documented in the medical record and the UDS Pro‑Doc system, in collaboration with the interdisciplinary team. Assists in the ongoing monitoring of identified outcome benchmarks for the inpatient population. Accesses confidential patient information and maintains all confidentiality of all information. What We're Looking For Applicants should have a Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program, or be a Licensed/Registered PT, OT, SLP from an accredited school. Required: Current state licensure such as Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP). Required: CPR/BCLS certification. Preferred: 3 to 5 years of experience in the care of patients in the acute hospital, skilled nursing facility or rehabilitation setting. Preferred: Excellent oral and written communication and interpersonal skills. Ability to work independently. Preferred: Flexibility with working up to 20 hours per week, including some weekends.
