Job Description
Title: Registered Nurse - Home Health

Schedule: 8am to 4pm, Monday - Friday, with some on-call and weekend requirements.

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 37

Requirements:
- OASIS experience required.
- Current nursing license.
- Knowledge of state and federal regulations.
- Proficiency in high-tech nursing skills (IV use and maintenance, complex wound care, etc.).

Roles & Responsibilities:
- Assess and address patient/caregiver needs using the nursing process to provide quality care.
- Develop and communicate a realistic plan of care in collaboration with patients/caregivers, adhering to evidence-based practices and regulations.
- Serve as the Case Manager for assigned patients, actively managing case loads and visit times.
- Complete OASIS documentation accurately and timely in accordance with agency policies and regulations.
- Implement nursing interventions and procedures to ensure quality and safe care.
- Evaluate the effectiveness of care plans and make adjustments as necessary.
- Coordinate care with interdisciplinary teams to ensure comprehensive patient support.
- Educate patients and caregivers on treatment options, medication side effects, and discharge instructions.

Compliance / Notes:
- Participate in monthly peer review audits.
- Follow agency protocols for documentation and communication with scheduling.
