Hospice Nurse Practitioner (NP)We are looking for a compassionate and experienced Hospice Nurse Practitioner (NP) to join our dedicated hospice team. In this role, you will provide high-quality, patient-centered care to individuals with life-limiting illnesses, focusing on comfort, dignity, and quality of life.The Nurse Practitioner will perform comprehensive patient assessments, develop and manage individualized care plans, and provide symptom and pain management. You will work closely with physicians, nurses, and the interdisciplinary hospice team to ensure coordinated care while supporting patients and their families during end-of-life care.Requirements:Active Nurse Practitioner licenseBoard Certification (AANP or ANCC)Minimum 2 years of hospice or palliative care experienceStrong clinical assessment and symptom management skillsCompassionate communication with patients and familiesAbility to work both independently and collaboratively with a care teamJoin a supportive team committed to providing compassionate, high-quality hospice care to patients and families in their time of need.Key Responsibilities:Conduct comprehensive patient assessments and develop individualized care plansManage pain and symptom control for hospice patientsOrder, interpret, and evaluate diagnostic tests as neededCollaborate with physicians and interdisciplinary team members to coordinate careProvide education and support to patients and families regarding disease progression and end-of-life careDocument patient care accurately in the EMR systemParticipate in interdisciplinary team meetings and care planning
