Physical Therapist - Full-Time

We are seeking a dedicated Physical Therapist for home health care, responsible for assessing patient needs related to mobility, endurance, pain, and medical conditions. Key duties include: Evaluating patients’ functional status and assisting in developing individualized treatment plans Providing therapy services within legal scope, in coordination with the healthcare team Monitoring and reporting patient responses and condition changes Implementing interventions to relieve pain, restore function, and maintain maximum ability Educating patients, families, and team members about care plans and progress Documenting visits, completing evaluations, and developing care plans per state guidelines Reporting outcomes and progress toward goals Qualifications include: Current state licensure and CPR certification Valid driver’s license and reliable transportation or access to public transit Ability to work effectively in diverse home environments Preferred skills: Multitasking and independent work abilities Strong communication, organizational, and writing skills Strong communication, organizational, and write...? #J-18808-Ljbffr Live Well Homecare
