PT - Physical Therapist

Outpatient Physical Therapist

This is a full-time position, 40 hours per week, Monday through Friday.

This is a 13-week contract position.

The Outpatient Physical Therapist is responsible for evaluating patients and developing, implementing, and modifying treatment plans according to their needs and goals. The therapist will also supervise therapy services for assigned patients and ensure effective functional treatment programs that facilitate return to community activities.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans focused on discharge goals within established lengths of stay.
• Document patient care details clearly, concisely, and accurately in a timely manner, ensuring compliance with necessary guidelines.
• Provide direction and education to patients, families, students, and coworkers as appropriate.
• Engage in a professional development plan aimed at enhancing skills and contributing to quality care.
• Participate in clinical improvement initiatives and provide feedback for improved service quality.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active state licensure in physical therapy.
