AMEDISYS HOME HEALTH
Amedisys, a leading home health and hospice provider, is seeking a registered nurse case manager to coordinate patient care in the Portland area. You will collaborate with the care team, perform patient assessments, develop plans of care, supervise LPNs and HHAs, and ensure timely documentation.
