Key Responsibilities\n

  • Utilize your educational background and clinical training to construct individualized psychosocial assessments and diagnoses for each patient.

  • Collaborate closely with patients to design personalized treatment plans that address their unique needs and goals.

  • Deliver behavioral health interventions using a variety of evidence-based techniques, such as solution-focused therapy, motivational interviewing, and behavioral activation.

  • Engage in cross-disciplinary collaboration with other mental health professionals, including psychologists and primary care providers, to enhance the delivery of integrated patient care.

  • Keep track of patient progress meticulously, whether in-person or via virtual sessions, to ensure therapy effectiveness and treatment adherence.

  • Facilitate coordinated management of psychotropic medications as prescribed by our medical team, concentrating on patient support, adherence compliance, potential side effects, and overall treatment outcomes.

Patient Population / Client Focus\n

Your work will primarily involve supporting individuals who seek mental health services, including those with diverse needs within community settings. This includes patients with chronic illnesses, those at the end of life, and their families who require around-the-clock emotional and psychological support.   • A Master’s degree in Social Work from a regionally accredited program is required.
