What You'll Do: Advocate for residents' psychosocial and emotional well-being
Assist with admissions, discharges, and care planning
Provide support and resources to residents and families
Coordinate community services and discharge needs
Work closely with the interdisciplinary team to ensure the highest quality of care
Help residents maintain dignity, independence, and quality of life
Qualifications: Bachelor's Degree in Social Work or related field preferred
Long-term care or healthcare experience preferred
Strong communication and organizational skills
Ability to work effectively with residents, families, and team members
Passion for resident-centered care and advocacy
