Physician Assistant (PA)

Physician Assistant STGi is currently seeking a Physician Assistant to provide services at our Community Based Outpatient Clinic (CBOC) in Kingston, New York. Job Summary The Physician Assistant delivers professional clinical services under the direction of the CBOC Primary Care Provider (PCP). As an Advanced Practice Generalist, the PA provides urgent and emergent evaluation of episodic and chronic problems, comprehensive primary health care, preventive health, medication prescription, laboratory and diagnostic test ordering, patient and family education, and participation in professional activities at local and state level. Essential Functions Provides primary and preventive medical care within scope of CPT codes and within the PA's training. Delivers medical patient care in accordance with national credentialing standards for PAs. Functions in accordance with the Physician Assistant Practice Act, Joint Commission standards, VA guidelines, and Inspector General directives. Works closely with the Patient Aliened Care Team (PACT) and support staff, including daily huddles. Emphasizes wellness, prevention, and early detection. Supervises support staff in patient care delivery. Maintains universal precautions and infection control practices. Ensures smooth and timely patient flow per VA access standards and triage protocol. Performs cardiopulmonary resuscitation (CPR) and assists during respiratory and cardiac arrest procedures. Investigates health problems by gathering and assessing historical information. Performs physical examinations appropriate to the patient’s history. Diagnoses health problems based on history, exam findings, and diagnostic results. Develops treatment plans tailored to patient needs, including health education and pharmacologic/non‑pharmacologic options. Prescribes medications as appropriate. Evaluates treatment results and modifies plans as needed. Provides referrals for specialty evaluation or treatment. Coordinates care for primary care walk‑in patients. Provides a clinical practice model within the clinic. Sets up and operates therapeutic and emergency equipment (monitor, EKG, oxygen, AED, portable suction). Obtains patient history and records findings per VA standards. Orders diagnostic tests per chief complaint, findings, and symptoms. Demonstrates knowledge of pharmacology and prescriptive authority. Recommends and follows up on laboratory and specialized diagnostic procedures. Documents all pertinent patient information in medical records and VISTA. Maintains VISTA clinic reminder tracking system for each enrolled patient. Closes patient charts within 24‑72 hours of encounter and promptly places Return to Clinic (RTC) orders. Assists in scheduling patients for initial and follow‑up appointments as per VA access standards. Keeps patients informed of their care plan and test results. Provides health educational materials and resources to patients and families. Assists the Registered Nurse to ensure accurate electronic documentation. Complies with all federal, state, local, Joint Commission, OSHA, VA, and subcontractor safety and operational regulations and directives. Participates in the ongoing Performance Improvement Program and peer reviews. Ensures required reports and clinical information are completed accurately and timely. Participates in staff development programs. Maintains confidentiality and supports patient privacy, rights, and safety. Aids in coordinating the CBOC's day‑to‑day activities. Performs other clinical duties assigned within limits of privileges. Required Skills A minimum of 2 years clinical experience as a PA in outpatient care in a Family Medicine or Internal Medicine environment (3‑5 years preferred). Current and unrestricted DEA license; must obtain a Scope of Practice/Collaborative Agreement with a state‑licensed physician. Current and unrestricted New York license, complying with all state requirements. Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification from AHA or MTN. Prescriptive authority per state rules and regulations. Ability to set realistic, measurable education goals for patients. Ability to provide individualized and group counseling on disease prevention and health maintenance. Experience with safe opioid medication practices for chronic pain. Proficiency with Microsoft Office and computer maneuverability. Excellent diplomatic communication skills and experience in handling difficult customer situations. Detail‑oriented and multi‑tasking ability. Required Experience Education Meets one of the following: (a) Bachelor’s degree from a PA training program certified by CAHEA; or (b) Graduation from a PA program of at least 12 months duration, certified by CAHEA, with a Bachelor’s degree in a health‑related science; or (c) Graduation from a PA program of at least 12 months duration, certified by CAHEA, with progressive health‑care experience such as independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health‑care experience must total at least five years. Maintain a full and current certification as a Physician Assistant from the NCCPA. Working Conditions / Physical Requirements Moderate physical activity; handling average‑weight objects up to 25 pounds and standing/walking for more than four hours per day. Work performed in a typical interior/office environment. Required to respond to any medical emergency within the clinic, including fast walking or job as necessary to provide assistance.
