RN Registered Nurse

We are looking for a dedicated **Registered Nurse** to join our team in the Bronx. Help New York residents achieve a better quality of living by conducting UAS assessments and providing leadership to our regional team of Nurse Evaluators. **The Basics** + **Pay Rate:** $60/hr + **Location:** Bronx, NY (3 days in community , 2 days remote) + **Hours:** Mon-Fri (8am-6pm) | Sat-Sun (10am-6pm) _Schedule flexibility available. 37.5 hours per week._ **Key Responsibilities** + **Field Evaluations:** Perform in-home patient evaluations in the Bronx area focusing on health strengths and care preferences via the **UAS-NY tool** . + **Team Support:** Act as a lead by instructing and observing new Nurse Evaluators during assessment visits to ensure high-quality service. + **Administrative Oversight:** Review consumer health referral forms and documentation for accuracy; transmit data into electronic forms promptly. + **Community Impact:** Facilitate enrollment into managed care health plans to ensure patients receive the support they need. **Qualifications** + **Licensure:** Valid RN license in the state of New York. + **Experience:** 2+ years of RN experience in home care. + **Technical Skills:** Prior experience using the **UAS-NY Community assessment tool** . + **Travel:** Ability to travel locally within the Bronx and surrounding area up to 50%. + **Home Office:** Reliable high-speed internet (min 20 Mbps download) and a personal cell phone.
