Physical Therapist

Role Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults. Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document patient progress in EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to enhance service accessibility and flexibility. Qualifications & Skills Valid PT license, degree from an accredited program, proficiency in EMR and basic computer skills, current CPR certification. Physical ability to perform tasks such as lifting, repositioning patients, and driving to patient locations is required. Key Duties Provide personalized, one-on-one care in various settings, support patient wellness, and promote functional independence through evidence-based interventions. Maintain effective documentation and work within an interdisciplinary team to optimize patient outcomes.
