Responsibilities

• Provide direct patient care under direction of the supervising RN, including monitoring and documenting vital signs.
• Administer prescribed medications and treatments per physician orders and facility policy.
• Assist residents with activities of daily living and mobility, promoting comfort and safety.
• Monitor for changes in residents’ condition and promptly report to the supervising nurse.
• Document care accurately in the electronic health record and participate in care planning with the interdisciplinary team.

Requirements

• Active Licensed Practical Nurse license in New Hampshire (in good standing)
• Graduate of an accredited practical nursing program
• Current CPR/BLS certification
• Prior experience in long-term care or skilled nursing facility preferred
• Ability to work evening shifts and meet assignment requirements, with competency in documenting care
