Job Title : PTA (Physical Therapy Assistant), Rehab, Home Health PT

Job Description:

The Physical Therapy Assistant will work in the Home Health setting, serving the greater Sacramento county area. They are responsible for treating a variety of diagnoses, including cardio, pulmonary, orthopedic, neurological, and geriatric patients. The productivity goal is five to six patients per day with guaranteed 40 hours per week.

Responsibilities:

Treat 5-6 patients per day in accordance with the productivity goal.
Submit personal timesheets showing mileage, signed by home health supervisors.
Participate in meetings twice a month for timesheet approval, or submit via email.

Requirements:

Two supervisory references required from the modality submitted (one within the last 12 months and one within the last 3 years).
Candidates should not have been directly employed by any CommonSpirit facilities, including CHI and/or Dignity, within the past year.
BLS (AHA) certification required.
