La Jolla Post-Acute is looking for a dedicated Occupational Therapist (OT) to join our rehabilitation team. The ideal candidate will help residents improve their daily living skills, independence, and overall quality of life through individualized therapy and patient-centered care.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services to improve daily living skills and functional independence.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers on treatment goals and techniques.
• Maintain accurate and timely documentation in electronic medical records.
• Collaborate with the rehabilitation team and other healthcare professionals to coordinate resident care.
• Follow physician orders, facility policies, safety procedures, and applicable regulations.
• Maintain required licensure, certifications, and professional development.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, active California Occupational Therapist license.
• Current CPR certification, as required.
• Long-term care or skilled nursing facility experience preferred.
• Knowledge of Medicare, applicable payor requirements, and resident rights.
• Strong communication, organizational, and documentation skills.
• Ability to manage multiple tasks and work effectively with an interdisciplinary team.
