Physical Therapist - Travel Contract

Join a travel Physical Therapist role in Elizabethtown, KY working in home health where you will help patients recover from injuries and manage chronic conditions while experiencing a new location. Minimum Requirements Master's degree in Physical Therapy required; Bachelor’s or Doctoral degree acceptable if meeting state licensure requirements. Active Kentucky Physical Therapy license required to start the assignment. Previous home health experience required. Experience using HCHB electronic medical record required. BLS certification from AHA or ARC may be required. Professional certification required as noted by state licensing. Preferred Qualifications Prior experience working for LHC or similar home health organizations preferred. Assignment Details Shift hours: Monday through Friday, approximately 8:00 AM to 4:30 PM. Weekly hour guarantee: 32 hours. Coverage area includes Elizabethtown, Hardin County and Hardinsburg, Meade County. Mileage reimbursement policy: reimbursed between visits from first patient to last patient, not to first or from last. No weekend or on‑call requirement. Impacting Patient Care The Physical Therapist in home health is responsible for assessment and evaluation of patient care needs related to functional status, mobility, endurance, pain, and medical conditions. The therapist develops treatment plans, provides interventions to improve patient outcomes, and evaluates progress. This role supports patient independence and functional recovery in community and home settings.
