RN Case Manager: Seamless Care & Discharge Planning

Indiana University Health is seeking an RN - Case Manager (PRN) for on-site work at the university hospital in Indianapolis. You will assess patients, coordinate care with a multidisciplinary team, and develop individualized discharge plans to ensure smooth transitions and appropriate post-discharge settings. Responsibilities include working with payers for coverage and authorizations, educating patients and families on post-discharge care, and ensuring compliance with Medicare/Medicaid #J-18808-Ljbffr
