Please click here to learn more about our locations.Job DescriptionWe are seeking a dedicated per diem Physical Therapist to join our team in a supportive, energetic, and patient-focused setting.The Physical Therapist (PT) provides evaluation and therapy services, including direct patient care, patient/family education and transition of care, in an efficient, age-appropriate, and culturally sensitive manner upon licensed independent practitioner referral.Key Responsibilities:Upon receiving a licensed independent practitioner referral, implements a systematic patient examination and evaluation utilizing comprehensive compliant and safe clinical care demonstrating the philosophy of family centered careWorks with medical staff and the multidisciplinary team to design and implement individualized plans of care to meet the needs of the patient and their family.Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team.Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals.Supports risk management and compliance initiatives through accurate, thorough documentation.Required Qualifications:Education necessary to obtain License in State of PracticeMinnesota PT licenseCPR/BLS (per hospital policy)Preferred Qualifications:1 year physical therapy experience; will consider new gradsPediatric experienceThe pay range for this position is $35.66 - $53.49.Compensation is determined based on years of relevant experience and departmental equity. #J-18808-Ljbffr
