Hospice RN - In-Home Care & Case Coordination
Affinity Health Management
Affinity Health Management seeks a Registered Nurse to plan, coordinate and deliver hospice care in patients' homes and communities. You will perform comprehensive assessments, develop and revise care plans, administer medications and educate patients and families. You will act as Case Manager when assigned and collaborate with the interdisciplinary team to ensure compassionate, goal-oriented care. Ideal candidates are NLN-accredited nursing program graduates, with current state RN licensure and #J-18808-Ljbffr Affinity Health Management
