Registered Nurse Case Manager (RN)

Job Location: Bristol Hospice - Rogue Valley, OR - Grants Pass, OR 97526 Position Type: FT Hourly Salary Range: $48.00 - $52.00 Hourly Job Shift: Any Schedule Four 10-hour shifts, Tuesday to Friday, 8 am to 6pm Coverage Area Jackson, Josephine, and Douglas Counties Culture Integrity: We are honest and professional. Develop and implement individualized care plans based on current and prior medical history. Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding. Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey. Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure. Administer physician-ordered medications and treatments with skill and empathy. Educate patients and their families on care techniques and symptom management. Track and communicate changes in patient condition, ensuring timely and effective responses. Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided. Collaborate with interdisciplinary teams to ensure comprehensive care delivery. Contribute to other supportive duties as part of a compassionate, mission-driven team. Perform additional duties as assigned. Qualifications A heart for hospice care and a commitment to exceptional service. A valid Registered Nurse license in good standing in the state where you will work. A valid driver’s license and reliable transportation. CPR certification (current). Strong observational and communication skills—both written and verbal.
