We are seeking a Licensed Practical Nurse (LPN) to support high‑quality, patient‑focused care in an outpatient clinic setting serving our senior population.

About the Role:
The Licensed Practical Nurse (LPN) provides direct patient care under the direction of a Registered Nurse or physician. This role includes assisting with patient assessments, performing clinical tasks, administering treatments as appropriate, and supporting daily clinic operations. The LPN plays a key role in delivering compassionate, coordinated care in a patient‑centered environment.

Key Responsibilities:
Provide nursing care in accordance with established standards, policies, and clinic protocols

• Assist with patient intake, clinical documentation, and care coordination
• Prepare patients for examinations and procedures
• Administer medications and treatments as directed
• Maintain accurate and timely documentation in the electronic medical record
• Communicate effectively with patients, families, providers, and interdisciplinary team members
• Support a safe, efficient, and welcoming clinic environment

Requirements:
Graduate of an accredited Practical Nursing (LPN) program

• Active LPN license in the state of practice or eligibility by endorsement
• Basic Life Support (BLS) certification from the American Heart Association
• Strong communication, organizational, and clinical skills
• Basic computer and EMR proficiency
• Ability to work effectively in a fast‑paced outpatient clinic setting
