Key Responsibilities

Provide outpatient physical therapy services to patients, with approximately 20% of visits conducted at assisted living facilities
Attend daily stand-up meetings at 8:30 AM, Monday through Friday
Manage a patient caseload ratio of 6–7 patients
Document patient care and progress using

Wellsky

charting software
Expected weekly mileage of approximately 50–75 miles for patient visits (may vary)
Maintain clean, professional scrubs and closed-toe shoes per dress code requirements
Required Qualifications

Active Physical Therapist license in good standing
Minimum of 2 years of recent licensed PT experience; no new graduates
At least 1 recently completed travel assignment required
Experience with home care and/or HomeCare HomeBase (HCHB) required
BLS and First Aid certifications required
COVID-19 vaccination and booster(s) required per state guidelines; medical/religious exemptions considered on a case-by-case basis and may require weekly testing
Must live at least 50 miles from the facility to be eligible
Preferred Experience / Skills

Prior experience in outpatient and assisted living facility settings
Familiarity with Wellsky electronic medical records platform
Comfort with regular travel between patient locations
