Advanced Practice Provider – Oncology, Survivorship Program

We are seeking a skilled and compassionate Advanced Practice Provider to join our Oncology team. The ideal candidate will have experience or a strong interest in Survivorship care and will work collaboratively under the supervision of multiple Oncologists and other healthcare professionals to deliver high-quality, patient-centered care.

Position Summary: We are seeking a skilled and compassionate Advanced Practice Provider to join our Oncology team. The ideal candidate will have experience or a strong interest in Survivorship care and will work collaboratively under the supervision of multiple Oncologists and other healthcare professionals to deliver high-quality, patient-centered care.

Key Responsibilities:

• Conduct comprehensive survivorship assessments including history, physical examinations, and interpretation of diagnostic studies.
• Develop, implement and regularly update individualized survivorship care plans following completion of cancer treatment.
• Summarize treatment history (including surgery, chemotherapy, radiation, and immunotherapy) and outline follow-up care
• Coordinate transition from active oncology care to long-term survivorship.
• Monitor for cancer recurrence and secondary malignancies
• Order and interpret appropriate surveillance labs, imaging, and diagnostic tests.
• Identify and manage late and long-term effects of cancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.
• Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.
• Counsel survivors on health promotion, disease prevention, and age/risk appropriate screenings and vaccinations
• Provide patient and caregiver education.
• Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.

Qualifications:

• Graduate of an accredited Physician Assistant or Nurse Practitioner Program
• National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
• Current Physician Assistant or APRN license in the State of Florida.
• AOCNP Certification
• BLS/ACLS
• Preferred Skills:
• 2 years of Oncology Experience
• Experience with Epic Electronic Medical Record
