Coverage area: Jackson/Sheboygan-(Washington/Ozaukee/Sheboygan counties)

Schedule: Full Time Monday-Friday 8AM-5PM

Allay Hospice is seeking a compassionate, grounded, and spiritually-sensitive Chaplain in Sheboygan, WI. Our Chaplains provide emotional and spiritual support to patients and families during end-of-life care. As a Chaplain You will: 

• Conduct and ensure timely spiritual assessments for patients and families 

• Participate in developing individualized care plans to address spiritual needs 

• Provides direct spiritual support to patients, families, and caregivers 

• Serves as a liaison to community chaplains and spiritual counselors 

• Maintains accurate documentation of spiritual services and communications 

• Attends interdisciplinary meetings and contributes to care planning
