Job DescriptionJob Description Job Title: Licensed Practical Nurse (LPN) – Home HealthPosition Summary The Licensed Practical Nurse (LPN) provides direct, hands‑on nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence, safety, and comfort while following individualized care plans and agency policies.Essential Duties and Responsibilities Provide skilled nursing care to patients in their homes according to the plan of careMonitor and document vital signs, symptoms, and overall patient conditionAdminister medications and treatments as prescribedPerform wound care, catheter care, ostomy care, and dressing changesAssist patients with activities of daily living (ADLs) as neededObserve and report changes in patient condition to the supervising RN or physicianProvide education to patients and caregivers on disease management, medication use, and safetyMaintain accurate, timely clinical documentation in accordance with regulatory requirementsFollow infection control, safety, and confidentiality standardsCollaborate with interdisciplinary team members to ensure coordinated careParticipate in case conferences, in‑service training, and quality improvement activitiesQualifications Education and LicensureGraduate of an approved Practical Nursing programCurrent LPN license in the state of practiceCPR certification (BLS)ExperiencePrevious home health or clinical nursing experience preferredExperience with geriatric or chronic care patients a plusKnowledge, Skills, and Abilities Knowledge of basic nursing principles and home health practicesAbility to work independently and make appropriate clinical decisionsStrong assessment, communication, and documentation skillsCompassionate, patient‑centered approach to careAbility to follow care plans and physician orders accuratelyBasic computer skills for electronic health recordsReliable transportation and ability to travel to patient homesWorking Conditions Work performed in patients’ homes and community settingsMay require travel, flexible scheduling, evenings, weekends, or holidaysAbility to lift, reposition, and assist patients as neededReporting Relationship Reports to: Registered Nurse / Clinical Supervisor
