Senior Case Manager, Residential Mental Health

Transitional Services for New York, Inc. (TSINY) is a not-for-profit mental health organization serving New York City. We seek a Senior Case Manager for our Bronx Residential Program to coordinate assessments, treatment planning, and care coordination for residents. The Senior Case Manager advocates for residents’ rights, helps set goals, links to community resources, and supports transition to independent housing.
