Case Manager – Beaumont & Houston Areas Location: Beaumont, TX (with travel to Houston) Company: Disability Services of the Southwest (DSSW) What You’ll Do Oversee day-to-day operations of the managed waiver program. Monitor and support services for your assigned caseload. Enroll new applicants and ensure compliance with DBMD program standards. Build strong, positive relationships with clients, families, vendors, and team members. Provide crisis intervention and on‑call support as needed. Travel to client homes and community activities in the Beaumont and Houston areas . What You Bring Education & Experience Bachelor’s in health/human services + 2 years’ experience Associate’s + 4 years’ experience High school diploma + 6 years’ experience Strong communication skills and fluency in client communication methods. Proficiency in MS Word, Excel, and database systems. Reliable transportation, valid TX driver’s license, and auto insurance. CPR/First Aid certification (in‑person only).
