We are a growing behavioral health group focused on supporting residents in skilled nursing and assisted living settings throughout Connecticut.
Our model gives providers flexibility, autonomy, and the ability to make a direct impact with patients who need consistent psychiatric support in a post-acute and long-term care setting.
This is a stable, flexible opportunity for a PMHNP who enjoys consultative work, geriatric psychiatry, and collaborating closely with facility care teams.

Job Details
This role is focused on psychiatric consults for patients in skilled nursing facilities and assisted living communities.
Responsibilities include:
Perform psychiatric evaluations and follow-up visits
Manage medication treatment plans
Collaborate with facility staff, physicians, nurses, social workers, and families
Provide consultative behavioral health support to post-acute and long-term care residents
Document visits and treatment plans in a timely manner
Travel to facilities throughout New London County
Qualifications:
Licensed Psychiatric Nurse Practitioner in Connecticut
PMHNP certification required
Experience with geriatric, long-term care, SNF, assisted living, or post-acute populations preferred
Comfortable working independently in the field
Strong documentation and communication skills
