Whether you serve patients directly or support those who do, your role helps bring out the courage in others.Deliver high-quality, compassionate care that supports patient health, safety, and emotional well-being.Provide social work services to home health or hospice patients under a physician's plan of careAssist the interdisciplinary team in understanding social, emotional, and environmental factors impacting careSupport patients and families with accessing community resources, financial support, and social servicesParticipate in the development and implementation of individualized care plansContribute to discharge planning to promote safe transitionsDocument all patient encounters, assessments, and interventions per CareM policyMaintain contact with community organizations and refer patients as appropriateParticipate in patient care conferences and consult with team members as neededAdhere to Medicare CoPs, state regulations, and professional social work standardsProvide care and services in accordance with infection control, safety, and risk prevention protocolsAccurately document clinical interventions, care coordination, and communication in a timely mannerParticipate in interdisciplinary meetings, case reviews, and continuing education opportunitiesMaintain compliance with HIPAA and all company privacy and confidentiality standardsRepresent the organization positively and professionally in patients' homes and community settingsWork independently with minimal daily oversight and demonstrate initiative in completing responsibilitiesProvide courteous, timely, and professional customer service to patients, families, and partnersExhibit adaptability to changing responsibilities and flexibility in completing assignmentsDemonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platformsAdhere to all organizational policies, including job descriptions, mission, and the Employee HandbookCommunicate and interact professionally and respectfully with others to support team goalsPrimarily works in the fieldExecutive Director - Home Health & Hospice, Director - Home Health, or Director - HospiceNonePatient care or physical work with injury riskQualificationsMaster's degree in Social Work from a CSWE-accredited programOne year of social work experience in a healthcare setting requiredLicensure or certification as required by the state of practiceKnowledge of community resources and psychosocial care planningStrong communication, assessment, and advocacy skillsProficiency in EMR systems; basic computer skillsValid driver's license, transportation, and auto insurance for home visitsMust be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
