Nurse Practitioner – Geriatric Primary Care Primary Care Provider for geriatric patients in private homes, assisted living facilities, and independent living facilities within a defined geographic territory in Cleveland, Ohio. Schedule Full‑time position 4‑day or 5‑day week No evenings, weekends, or mandatory holidays Compensation & Benefits Annual income: $125,000–$170,000 Base salary: $105,000–$125,000 Annual bonus: $20,000–$45,000 (paid quarterly) Malpractice, AD&D, STD, LTD insurance Health, dental, and vision insurance Paid time off CME allowance License/DEA fee reimbursement 401(k) plan Mobile‑based EHR with AI functionality Location Cleveland, Ohio – patients located in private homes, assisted living, or independent living facilities. Position Description Responsibilities include the following: Conduct health assessments to identify patient needs Develop personalized care plans and regularly evaluate patient conditions Adjust treatment plans and perform minor procedures and wound care as required and permitted by the setting Educate patients and families on health management, disease prevention, and lifestyle modifications Coordinate care with physicians, nurses, and other health professionals Utilize a mobile EHR to reduce administrative time Participate in scheduled phone triage as on‑call rotation Requirements Valid Ohio Nurse Practitioner license DEA registration New graduate experience in home health, long‑term care, or related settings encouraged No previous experience required; full training provided #J-18808-Ljbffr CHS Recruiting
